Grace Care Services
Grace Care provides two basic types of service to our clients; Companion/Homemaking Services and Personal Care Services.
Companion/Homemaking Services include tasks of daily living that do not require hands-on assistance to clients such as light housekeeping, meal planning and preparation, shopping, errands, laundry, home organization, accompaniment to appointments/meetings/activities, medication reminders and companionship. Clients receiving this level of service are generally in good health with no need for assistance with mobility, continence or hygiene.
Personal Care Services are those types of assistance requiring more hands-on aid from caregivers such as bathing/showering, grooming, hygiene, dressing assistance, continence care and mobility assistance/transfers. Often clients receiving personal care services may be recovering from an illness, injury or hospitalization and may possibly be receiving coordinating services through Home Health or Hospice. Grace Care provides licensed Home Health Aides who are well trained in providing this more specialized level of care safely and compassionately, while focusing on enhancing each client's sense of independence and dignity.
At the beginning of service, an assessment will be performed by a Registered Nurse for each client. This nursing assessment will be used to develop an individualized plan of care that takes into consideration the client's physical and medical situation as well as their specified wishes concerning tasks to be performed and preferred schedule.
